The metal Scandium (Sc) is a transition element yet it forms merely one ion with a +3 charge. Why is this true and why would scandium not form a +4 charge ion?

Look at the electron configuration:

[Ar]-4s2-3d1

It can single lose 3 electrons before it obtains a "noble" configuration. The dynamism required to remove the additional electron from the Argon octet to give a +4 state is too excessive for typical chemical interactions.
The most common oxidation state of scandium in is +3. Scandium chemically resembles yttrium and the exceptional earth metals more than it resembles aluminium or titanium. Thus scandium is sometimes seen as the scandium oxide, Sc2O3, and as scandium chloride, ScCl3.

Sc is not a transition element. its with the sole purpose a d block element.

a transition element is one which forms compounds near ions thats has a partially chock-full d subshell

so when sc forms an ion, it loses all 3 e- to form a stable octet config and so theres no partially chock-a-block d subshell since theres no e- in d subshell at all

similarly for zn,
it loses 2 e- to form a stable octet config but since theres a completely packed d subshell, theres no partially filled d subshell too

thus sc and zc are not transition elements but merely d block elements

as for the other transition elements, they can lose adjectives or some of their valence 3d e- and so they exhibit mulitple oxidation states due to the similarity in energy level of a 3d and 4s electron
